This form is for minors (accompanied or unaccompanied by guardians) who are participating in trips that are led by an NPH USA staff member.

Minor Notarized Document

Please note: Guardians of minor participants must complete both the DocuSign Minor Trip Packet (sent to you via email by the National Trip Coordinator) in addition to the above notarized document form. The Notarized Form must be mailed/handed to the group’s Trip Leader a month before the start of your trip. Please contact your Trip Leader or the National Trip Coordinator at travel@nphusa.org for more information.
